Transitional Concurrent Care (TCC) helps patients smoothly transition to hospice care. It includes curative treatments, for up to 60 days, after choosing hospice care with an in-network provider, and is coordinated between the TCC provider, hospice team, and the VNS Health care team, as applicable. These services include any treatments or care planned for the patient’s terminal illness with their current healthcare provider. TCC offers a range of services, on a short-term basis, to support patients with different medical needs. For cancer patients, we cover treatments like radiation for pain management, blood transfusions, and essential services like IV antibiotics, and total parenteral nutrition (TPN). Cardiac patients may receive care including diuretics, nitrates, opioids for comfort, and physical therapy, depending on their symptoms. In cases of respiratory issues, treatments are determined on a case-by-case basis and may include chest tubes, CPAP/BiPAP, ventilator support and oxygen therapy. Those with Chronic Kidney Disease might receive dialysis for palliation, with coordination of ongoing care if needed. This list is not exhaustive; we consider each request individually, ensuring each patient receives the most appropriate care for their condition.
