  • Health Home - Adults Comprehensive care coordination integrating medical and behavioral health service needs for adults (age 21+) with mental health and/or substance use conditions, HIV/AIDS, and/or multiple chronic illnesses. Active Medicaid required.
    Health Home - Children Comprehensive care coordination integrating medical and behavioral health service needs for children between the ages of 6 and 21 with behavioral health conditions and/or multiple chronic illnesses. Active Medicaid required.
    Older Adult Supportive Counseling  Free in-home therapy for older adults to address depression provided by funding from New York City Council.
    Children and Family Support Team Comprehensive assessment and short-term crisis intervention case management, and family support for children and youth with significant emotional and behavioral needs.
    Integrated Mental Health and Outpatient Substance Use Clinic Comprehensive, holistic care providing assessment, treatment, and medication management for children and their families. Services include outreach, screening, care coordination, and recovery support, addressing both mental illness and substance use disorders.
    Children's Home-Based Crisis Intervention Short-term, in-home services for families with children or adolescents who are at risk of hospitalization due to serious mental health conditions.
