    VNS Health needs a referral to start home care services, so your first step is to talk to your health care provider. They can use our online referral form, our downloadable referral form (which can be faxed), or a provider platform such as HCHB, Focura, or CarePort.

  • VNS Health offers training for qualified people interested in becoming a certified home health aide. To learn more or to apply for training, you can:

    1. Visit our training center at 5 Penn Plaza, 20th floor, Monday – Friday between 9:00 am and 3:00 pm.

    2. Call 1-212-609-4442, option 4, for information or to schedule an appointment with a recruiter. Appointments are available in Brooklyn, Staten Island, and in Nassau, Suffolk, and Westchester counties.

    To qualify, you must:
